NDH002 - % patients with NDH who have had a HbA1c or fasting glucose performed in the last 12m
NDH or pre-diabetes is designed to intervene to prevent or delay the onset of type 2 diabetes. NDH has the fastest growing prevalence rate; 1.07% increase in the last comparison with more than 5 million people living with NDH.
The idea of this register is patients who are pre-diabetic are monitored annually. However, many practices we work with do not put patients onto the register until they have 2 raised HbA1c’s.
Patients on the register are called in each year and as with all QOF registers, if they do not respond after 2 invites, they are ‘excluded’ from your target patient list. Also, having an accurate register means the ‘value’ of these 18 points is much more accurate - you are being paid for following the patient up and monitoring them.
However, if they are not on the register after their first high result, how do you monitor them? Do you tell the patients they need to contact you to book a repeat test? How many of them do this? And how many don’t?
For those who do come in for their repeat test to see if they do need to be added to the register, essentially you are not being paid for this work - they are not on the NDH register so this is ‘free’ repeat work you are carrying out.
Our QOF Health Checks generally identify the most patients being missed off this register. And on the basis this register is all about delaying or preventing the onset of T2 diabetes, leaving it to chance seems a little risky.
