Support for older people living with frailty is one the 3 areas the NHS faces ‘particular challenges’ with. Frailty can help predict who is likely to have a fall, become dependent on others or experience unplanned admissions to hospital or a care home. It can also be associated with anxiety, depression and poor quality of life.
Practices will use an appropriate tool, e.g. Electronic Frailty Index (eFI) to identify patients 65 & older who are living with moderate or severe frailty.
Those patients who are, practices will:
- Deliver a clinical review including annual medication review
- Where clinically appropriate discuss falls in the last 12 months
- Provide other clinically relevant interventions
- Where patients do not already have an enriched Summary Care Record (SCR) practices will promote and seek informed patient consent to activate SCR
