The Exception Manual

The concept of exception coding has not changed since QoF was introduced in 2004, however, many practice still do not fully understand when it is appropriate to use them. High use of exception codes does not necessarily mean that you are not using them correctly, however, it could be disguising a bigger problem - high usage will alert your PCT/LHB to start asking awkward questions to which you may not know the answers. Many codes are self explanatory, such as patient unsuitable - but what constitutes a patient as unsuitable? How many really understand the true meaning of informed dissent? Three letters just asking the patient to make an appointment to attend the asthma clinic is not informed dissent and it is not going to make the patient feel that it is in their best interests to attend - after all, that is really what we are trying to do, exceptions are only there as a last resort rather than a preference!

Patient Participation DES Managers Pack

This is a 2- year DES applicable to England Practices only and effective from 1st April 2011 - 31st March 2013. The purpose of the DES is for patients & Practices to work together to improve services & to involve patients in decisions about the range and quality of services provided - encouraging patients to take an active interest in their own health. Practices will be rewarded for routinely asking for and acting upon the views of the patients. This DES aims to promote the pro-active involvement of patients through the use of Patient Reference Groups (PRG'S) and local surveys. The outcomes and the views of patients must be published on the Practice website within the given timescales.

This template allows practices to quickly and correctly read code QOF disease diagnoses - consistent data entry = better quality patient care & practice income. This is perfect for clinicians in ‘Consultation mode’ and for summarisers in ‘Medical record’.

Once installed, the template will display all of the QoF disease areas - once a diagnosis is selected a sub-template will be displayed so that an accurate diagnosis can be recorded using a ‘QoF approved’ read code.

This template is based on V20 business rule sets (current version).
